Why was I chosen to participate? / Why did I receive a questionnaire in the mail?
CRI/Kantar Media is conducting an independent research initiative designed to understand the healthcare concerns, lifestyle, and media use of people like you. Every year, we randomly select a limited number of individuals across the U.S. to represent thousands of American consumers. This year, you were chosen to take part in this important research.


How did you get my name and address?
You were randomly selected from a national database with contact information that is collected from publicly available sources, such as telephone directories and other public records.


Why are you sending me money?
The money in your survey package is a token of our appreciation for your participation and prompt response.


Is the cash in the survey package real?
Yes, the money is real and yours to keep. We appreciate your input and the money is a token of our appreciation for your timely participation.


Why are there two questionnaires in my package?
As part of our research, we try to include many different types of individuals. Your survey package may contain both English and Spanish versions of the questionnaire. Please complete and send back the version you feel most comfortable filling out. The other survey can be thrown away.


I don’t speak Spanish. Why did I receive a Spanish letter/questionnaire?
As part of our research, we try to include many different types of individuals. Your survey package should also contain an English version of the questionnaire. Please complete and send back the English version. The Spanish survey can be thrown away. Note that we make two attempts to mail questionnaires; first in early January and again in mid-February. If you did not receive the first English questionnaire, please keep an eye out for your new survey package.


Do I have to buy anything?
No, this is not an attempt to sell you anything. Our study is being conducted for market research purposes only. CRI/Kantar Media is not involved in telemarketing or sales, and survey participants will not be identified for any marketing or sales efforts as a result of taking part in our research studies.


Will I end up on mailing lists if I participate in your study?
No, survey participants will not be identified for any marketing or sales efforts as a result of taking part in our research studies.


Can I do this survey online?
Our study is currently only available on paper. We hope you will take just a few minutes to fill it out and send it back in the postage-paid envelope provided.


I don’t feel comfortable answering some of the questions.
All of the questions are important to the quality of our research and we take privacy very seriously. Your answers will be combined anonymously with responses from other survey participants so that no specific individual or household is identified. What does CRI/Kantar Media do with the information collected in the questionnaire?
Responses from this study will be used to create statistical data to calibrate information collected in online research tools. Can I trust CRI/Kantar Media with my personal information?
CRI and Kantar Media have been conducting consumer research in the U.S. since 2001. Our ability to collect accurate information on consumer preferences and attitudes depends largely on earning the trust of our respondents, so safeguarding your personal information is a primary responsibility of our research team. All personal information provided to CRI/Kantar Media is combined and reported anonymously with information gathered from other survey participants; no specific individual or household is identified. When should I mail my completed questionnaire back to you?
We would greatly appreciate it if you would fill out your questionnaire and mail it back right away. The questions should only take about 5 minutes to complete.


Why is there a barcode on the questionnaire?/How will you know the questionnaire is from me?
Barcodes that can be scanned are the most efficient way to keep track of which households have returned their completed questionnaires to us. Using barcodes rather than names/addresses helps us ensure your privacy and also avoid sending unnecessary reminder letters.


I have a friend/family member who did your survey. How can I participate in your study?
We appreciate your interest! Unfortunately, for this study we can only include the opinions of individuals who were randomly selected to participate and received a survey questionnaire in the mail. Our participant selection process involves very specific scientific methods and we cannot substitute or add any other individuals.


I lost my survey questionnaire. How can I get a new one?
We make two attempts to mail questionnaires; first in early January and again in mid-February. If you misplaced the first questionnaire, please keep an eye out for your new survey package.


I lost the postage-paid return envelope. Where should I send my questionnaire?
